A new Cochrane review details the lack of evidence for antidepressants in the treatment of chronic pain.

The message in journal editorials, comments and opinion articles, is that 'this new study shows great promise' and that 'we need further research'. My interpretation is: 'give us the money and we will be happy to carry this out'. With the implied promise that, once this new research has been done, we will get a better world. Sadly this is rarely ever the case.

Long-term opioid prescribing has not only been shown to not be helpful for chronic pain; it in fact worsens pain by repeatedly causing tolerance and withdrawals (the main symptom of which is pain). This is analogous to how psychiatric drug use, though often helpful initially, ultimately can cause people to become chronically “mentally ill.”

The quickest way to restore safe use of opioid prescription is to insist that the drug companies that promoted the overuse of opioids now create a pot of money to develop powerful TV, radio, and print ads, free continuing education offerings, and drug rehabilitation research.

Opioids are now the leading cause of mortality from overdose, accounting for 91 deaths every day. The context and key players that created and contributed to the opioid epidemic must be brought into sharp focus if we are to have any hope of stemming the tide of this public health crisis.
